Physical therapy claims are easy to get wrong. Most treatment codes are timed, Medicare counts units using the 8-minute rule, and modifiers decide whether services billed on the same day get paid. I take care of the billing so your therapists can spend their time with patients.

Physical therapy billing FAQ
What is the 8-minute rule?
Medicare counts timed therapy services by adding up the total timed minutes in a visit: 8 to 22 minutes is one unit, 23 to 37 minutes is two, 38 to 52 minutes is three, and so on. Some commercial payers count units differently, so each payer’s rule is followed. See the 8-minute rule explained.
Which modifiers do PT claims need?
Medicare therapy claims carry the GP modifier. The KX modifier is added when medically necessary care goes past the annual therapy threshold, and modifier 59 or an X modifier may be needed when two normally bundled codes are both justified by the documentation.
Do you track authorizations and visit limits?
Yes. Authorization needs and visit limits are checked before care starts and tracked through the episode, so visits are not lost to expired approvals.
